JPEG files are generally going to be the easiest to work with (e.g. if you want to add the monogram to a word document, make a page with multiple copies of the monogram). The PDF file would be a better choice if you have access to design software like Adobe Illustrator. You'll be able to print a PDF using Adobe Reader, but you won't be able to insert it into a word document. You'll be able to upload the JPEG file to a website or online photo sharing account, but not all will accept a PDF file.
PNG files are similar to JPEG files, but have a transparent background.
If you'll be using a vendor to print the monogram, you should check with them to see what file type they prefer.
For the file size: Somewhere around 3" is recommended for most DIY projects. You will be able to make slight adjustments to the size of the design when you receive your final file (about .5" larger or smaller) but keep in mind that making drastic changes to the size will result in loss of quality.
