The ear cuff itself slides down the ear to sit above the lobe. I have worn ear cuffs for 25 years and NO, they do not hurt! It doesn't pinch at all, yet it stays put.
This earcuff hangs less than 2 inches (5 cm) from the silver-plated cuff. The shell itself is about 1.5 cm (over 1/2 inch) long.
