Is this for the supervisor or the supervisee?
This kit is for the SUPERVISOR — the credentialed BCBA who is providing supervision and needs to document their own records: the supervision agreement they sign, the observation notes they write, the monthly attestations they co-sign, the competency ratings they complete, and the final verification they attest to. If you are the supervisee tracking your own fieldwork hours, this is not the right kit.

Do the forms meet BACB requirements?
These are blank documentation shells; they are not compliance certification. BACB supervision requirements — including training requirements, format, frequency, observation minimums, supervision percentage, and verification procedures — are set by the BACB and change over time and by credential pathway. Can I use the same forms for all my supervisees?
Yes — each form is blank and intended to be completed fresh for each supervisee relationship. The forms are general-purpose across BCBA candidate, BCaBA candidate, and RBT supervision contexts.

Does this replace the supervisee's hours log?
No. The supervisee maintains their own hours log in the BACB's system or equivalent. This kit is the supervisor's independent documentation of each contact and evaluation. The two records should be consistent and reconciled — file 09 (Supervision Contact Log) is designed specifically as the supervisor's companion record.
