Every unit sheet carries the same core card, which is room and bed, name or initials, age and sex, code status, isolation, admit date, MD or team, allergies, diagnosis and history, diet, activity, the six systems (neuro, cardio, resp, GI and GU, skin, pain), IV and lines and drains, labs due, a medication and times grid, a to-do list, notes and an SBAR block. Med-Surg then adds last BM and void, fall risk and precautions, blood glucose and insulin, wound and dressing, and the discharge plan. Each unit block below is described at its fullest; the tighter layouts carry the same boxes in fewer words, and at 6 patients per page the unit block is one line.
02 - Telemetry report sheets (20 files)
The core card plus rhythm, rate, QTc, telemetry box number, potassium and magnesium, pacer or AICD, anticoagulation with room for the drug, the dose and the last INR or anti-Xa, and a line for rhythm changes and events.
03 - ICU report sheets (20 files)

The core card plus weight in kg, height or length, caregiver and contact, developmental stage and needs, a weight-based dosing grid with the drug, mg per kg, the dose, the maximum and the route and frequency, comfort and distraction, safety for crib and rails and sitter, and immunizations and school.
07 - Night Shift report sheets (20 files)
The core card plus a rounding check row across the night hours, a PRN grid with the med, the time, the reason and the effect, sleep, fall risk and bed alarm, and a line for the 04:00 labs. The medication grid is timed for nights: its columns run 19:00 to 06:00 on the one patient page rather than 07:00 to 18:00.
08 - SBAR handoff sheets (8 files)
SBAR-Handoff-Sheet-1-Patient-Color-Letter.pdf and the four patient version, both styles, both paper sizes. The one patient sheet gives Situation, Background, Assessment and Recommendation a deep box each with the prompt printed inside it, under a patient line that carries the diagnosis and a full set of vitals, and over four pending items and a box for the questions you want to ask the oncoming nurse. 🔧 HOW IT WORKS

1. Open the folder for your paper size, then your unit, then the number of patients you carry. Print it at 100 percent, or click into the first box and type. Color prints in the unit's own tint; Ink-Saver is the same sheet in black at hairline weight.
2. Open the workbook and type your shift into the amber cells. The sample is nurse Sam Delgado, RN, a day shift on Monday September 14 2026, 07:00 for twelve hours, four patients in six slots by initials, with the time now typed as 07:45. Med Pass carries 18 medications and 20 scheduled doses inside the shift. NEXT MED DUE reads 08:00 metoprolol tartrate 25 mg PO, with 302 J.R., 15 minutes from the time now under it; DUE THIS HOUR reads 2 and DOSES LEFT reads 20. Change that first dose time from 08:00 to 09:00 and the headline becomes 08:30 pantoprazole 40 mg IV, 305 M.K., 45 minutes from the time now, and DUE THIS HOUR drops to 1. The next thing due is now a different drug for a different patient in a different room. Type 19:00 as the shift start instead and every due time, every hour of the shift clock and every sort order wraps through midnight on its own. Every name and every patient in the sample is invented.
3. Work the shift. Twenty timed tasks sort themselves so the next ten undone ones stand in time order. Vitals holds a q4 grid for six patients, seven measures at four sets each, and flags anything outside the low and high numbers you typed. At handoff, SBAR Builder pulls the patient's facts across from Patients, takes your four boxes and assembles four paragraphs to read out loud.

What format is this? 156 fillable PDF sheets, which is 39 sheet designs drawn once in Color and once in Ink-Saver and set for both US Letter and A4, plus one workbook (.xlsx) in four color editions. You get a zip file to download the moment payment clears. There is no Canva file and no Word file.
What do I need to open it? Any PDF reader for the sheets, and Excel or Google Sheets for the workbook, on a computer, tablet or phone. There are no macros and nothing to install. For Google Sheets, upload the .xlsx and use File, Import, Upload, Replace spreadsheet. If a browser preview will not let you type into a field, or loses what you typed when you close it, open that PDF in Adobe Acrobat Reader, in Preview on a Mac or in Edge instead. Typing is saved only when you save the file.
What does the workbook compute? Every dose time on a med row from the first dose time and the frequency, how many of those doses fall inside your shift, how many are still to come, how many are due in the next hour, and which med in which room is next. Also the hours into the shift for any time you type, a twelve hour shift clock with the doses in each hour, the next ten undone tasks in time order, a check flag on any vital sign outside the thresholds you typed, and four finished SBAR paragraphs for the patient you pick. Change the shift start to 19:00 and every one of those re-times itself around midnight.
Is any of this medical advice? No. Nothing here checks a dose, a route, an allergy or an interaction, and nothing here is a medication administration record or a medical record. The sheets give you boxes to write in, and the workbook does arithmetic on the times you type. Your orders, your facility's MAR and policy, and your own judgment come first, every time.

These sheets and this workbook are for a nurse's own organization during a shift. They are not medical advice, and they are not a legal record or a medical record. Nothing here checks a dose, a route, an allergy or an interaction, and the workbook is not a medication administration record.
No patient information is stored in any of these files. There is no account, nothing is sent anywhere, and nobody but you sees what you type. If you type real patient information into your copy, that copy is protected health information from that moment on: keep it where your facility says it belongs and delete it the way your facility requires. Anything you write on paper that carries patient information is handled and destroyed under your own facility's policy, which on most units means shredded in a locked bin before you leave. The sheets are built for initials rather than names, and so is the workbook.
The sample is invented. Sam Delgado is not a real nurse, and J.R., M.K., T.A. and L.B. are not real people: they are four made-up patients identified by initials only. No real patient information was used anywhere in this product.
BID is spaced as every twelve hours and TID as every eight. Most pharmacies set standard administration times that are not evenly spaced, so use q4h, q6h, q8h or q12h when you need the exact spacing your facility uses. Each med row schedules up to six doses, which is more than a twelve hour shift at q4h ever needs. The workbook holds six patient slots, 24 med rows and 20 to-do rows, so a seventh patient needs a second copy of the file. The vitals low and high numbers are yours to type, because an 81 year old two days after a hip repair and a healthy 25 year old do not share one set, and the flag is a typing check rather than a clinical one. Print at 100 percent, or Actual Size: scale-to-fit shrinks the handwriting rows below what the layout was measured at. The four color editions are four separate files, so a time typed into the Light copy does not appear in the Dark one. Ink-Saver still prints every rule, in black at hairline weight rather than tinted, so it is a sheet with no fills rather than a toner-free sheet. Apple Numbers opens the workbook but drops some formatting.
