Food Allergy Chef Card Printable, 6 Language Restaurant Translation, Fillable Travel Allergen Wallet Size

Fillable food allergy chef cards in 6 languages, to hand to your server and chef when you eat out on a trip.

I'm Shawn. On our 2024 trip along the Amalfi Coast with our 15-month-old son, who has severe food allergies, finding safe food was the hardest part of every day. We looked up restaurants ahead of time, then asked again once we sat down. These cards are what I wish we'd had in our bag for that second conversation.

LANGUAGES
English • Italiano (Italian) • Español (Spanish) • Français (French) • Deutsch (German) • Português (Portuguese)

WHAT'S INCLUDED
• 1 fillable PDF file, 11 pages, US Letter (8.5 x 11 in), also prints on A4 with "fit to page"
• Full-page chef card in each language: a fillable name, a checklist of the 14 major EU allergens with an English gloss under each term, 2 "Other" lines and a clear cross-contact request
• Fold-over wallet cards (about 3.5 x 2 in folded) in all 6 languages, with a fillable name and allergen lines
• A Quick Questions page with 5 phrases to point to (oil, pans, "can you make it without...?") in Italian, Spanish, French, German and Portuguese
• A how-to-use page

HOW IT WORKS
1. Purchase and download the PDF instantly from your Etsy account (Purchases & Reviews).
2. Open in a free PDF reader (like Adobe Acrobat Reader), type the name, tick the allergens and save.
3. Print, laminate or tape, and keep the cards with your travel documents.

PLEASE NOTE
• These cards are a communication aid, not medical advice. Talk to your allergist about your travel plan, and always carry the medication your doctor prescribed.
• The allergen terms follow the standard EU allergen list, and were checked against the official EU wording. Always confirm with restaurant staff, and keep anything you type into the "Other" lines short and simple. Portuguese uses Portugal (EU) terms, with Brazilian alternatives in brackets.
• No card can guarantee a meal is safe.
• This is an instant download. No physical item will be shipped.
• For personal use only. Please do not resell, share, or distribute the files.
