The Elder Thing itself is based off an unused sketch from the process of making THE SISTERHOOD cover art. It's meant to have a medieval style and not be 100% faithful to Lovecraft's description. Anatomical accuracy is not a hallmark of illuminated texts or woodcut prints of that period.
