Original Antique St. Louis, Mo PROHIBITION Whiskey PRESCRIPTION for Medicinal Liquor on April 28, 1933 for G. B. Fox..
Here's an authentic Original National Prohibition Whiskey Prescription Form (White Form not pink Duplicate Form). This Original white form was to be forwarded by the Druggist to the office of the supervisor of permits..This was printed by Treasury Department U.S. Internal Revenue and is a Prescription Blank by the National Prohibiton Act as printed at bottom of Rx and on sides...
When you hold this up to the light (pic #3) you can see "Industrial Alcohol" printed, watermarked in the paper, and the Treasury Seal in Latin in the center, and informative directions on back...

The Alcohol prescription is for 16 oz. of Spts. Frumenti which is the latin for Whiskey and directions are or look like Tablespoonful 3 Times a Day...(in Pharmaceutical lingo) .. This was a way of getting whiskey, liquor, booze, during the prohibition days by having your Doctor prescribe it for medicinal purposes...(Dr. Grosby in this one)
