A great original 1936 issue Treasury Department order form for Opium, Coca Leaves, or Compounds, Manufactures, Salts, Derivatives or Preparations! A similar form was recently shown on the History Channel during a program on illegal drugs. The governent used a certain dye to make this form look old. Please see photos!

The form is ornate with a beautiful poppy graphic on the top corner. It was issued to Dr. Jarrett E. Williams of White Pine, TN. An order for two types of Morphine, Atropine, Codeine, ASA & Codeine Papine and 1/4 oz of Cocaine was ordered on June 27, 1946. This red duplicate was kept by the pharmacist or doctor for his/her records, the top original order form in black, was sent to Albers Drug Co., in Knoxville, TN. The order form is large, 10 3/4" by 8 1/2", has one corner bump and two tiny spots on the bottom border, otherwise, condition is EXCELLENT...great in person...one of the nicest I've seen!
