A modern french medal depicting Our Lady of 3 Ears of Wheat.

The fantastic art is likely the work of the great French artist Fernand Py (1887–1949), although the only signature on it is an S with a cross or t on top. I do know that medals with this hallmark were made by the JB foundry in France. So this may be an uncredited Py design. While it does contain design elements found in other Py works, this could also be the work of another artist who designed for JB after Py, who signed his work (when he signed at all) P. Cham.

The story of the apparition :

On May 3, 1491, Thierry Schoéré, was going to the market of Niedermorschwihr (Alsace) passed in front of a oak tree where he saw that somebody had placed a holy card in order to invite the passers by to pray for a man who had just passed away in this place a few days before.
He knelt in front of this pious image and prayed fervently for the soul of the deceased, when suddenly he was dazzled by a bright luminous beam of light, and saw in the middle of it a delicate and vaporous form. It was the Virgin Mary holding three ears of corn in her right hand and a small piece of ice in her left. “Stand up, my good man,” said the Virgin softly, “and listen to me. Do you see these ears of wheat? They are the symbol of the abundance of the beautiful harvests that will come to reward the virtuous and generous inhabitants of this region. As for this piece of ice, it signifies that hail, frost, floods, famine and all its succession of desolation and misfortunes will come to punish the non-believers whose gravity of sins have wearied the Divine Mercy. Go, my good man, go down in the villages and announce to all the inhabitants the meaning of these prophecies."
The man went along his way, but he feared mockery, and eventually decided to keep silent. When he arrived at the market, he bought a bag of corn, but, strangely enough, neither he nor anybody else could pick it up. The bag seemed to be made out of lead and fastened to the ground. The peasants began to worry about the witchcraft and the mysterious force that riveted the ground bag. This unexpected phenomenon tormented the crowds who started to accuse the unhappy blacksmith of black magic.
Then Thierry Schoéré finally understood the reason for this magic spell as a warning signal. As a divine messenger, he had disobeyed the Holy Virgin and had not fulfilled the mission that she had entrusted to him. In front of the crowd, which had suddenly gone quiet, Schoéré fell to his knees, and asked the Virgin Mary forgiveness. He finally managed to fulfil his mission, and he shared with the attentive inhabitants the celestial apparition. He cordially explained the symbolism of the piece of ice and three ears of corn.
Amazed and filled with admiration, the inhabitants listened to the story respectfully. The assistance was deeply moved and no one dared to question the sincerity of the honest blacksmith of Orbey. The most incredulous were conquered and expressed a sincere repentance and swore to make amends.
Then, relieved and happy, Thierry Schoéré went back to his mysterious bag. It was a miracle! Hardly had he touched it that he could pick it up as easily as a bag of feathers and charge it at once on his horse. Afterwards, amidst cries of delight from the assistance, Thierry Schoéré, the messenger of the Virgin, went joyously back towards his native village.
