TO OPEN THE .ZIP FILES
+ Mac: Simply double click the .zip, and your computer will create a regular file right next to it!
+ Windows: Right click and select "extract all". Follow the instructions from there!

The PDFs and Excel documents can be printed in order to fill out by hand. They are also set up to be electronically form-fillable, which means you can fill this out on your computer and print it, saving you any potential hand-cramping. How sweet is that?!?

These forms can be printed on any standard US letter-sized stationary (8.5" x 11"), from plain white paper, to colored sheets, to card stock.
