I love how adaptable this contact is, as well as how comprehensive it is. I've seen few others that include internet safety guidelines. It has prompted some long overdue conversations for our family, and helped us put in place some healthier smartphone habits.

So helpful and clear! I used this form to prepare for a trip my 14 year old son is taking without parents. It contained information and permissions that I wouldn’t have known I needed, and am grateful to have had spelled out for me. The form was easy to insert our personal information into, and now I feel well prepared and less anxious about sending my son off. Thank you!!
