Quick question

When you mark an item as shipped, does the customer get a notification or an email? Anything like that?

I sometimes mark them in batches long after the items have been sent and received, and feel awkward doing it so in case the customer gets notified.

No, they don't receive anything unless you choose to send notification. (But the date shipped appears in the Purchases section of their account.)
No, not unless you manually click the "send notification" button. I mark mine shipped but use different notifications, so I know you can just do that. Of course, the date of shipment will show wrong, but that's probably not a big deal for you since you don't physically ship anything.
No, you have to either ship through PP, or send a notification from the Etsy page where you mark it shipped.
