Her name is Opal. She is sweet, feminine, and oh so pretty.

She features a vintage music sheet bodice accented with vintage buttons and lace. Her tiered skirt is done in a beautiful soft blue. A gorgeous lace trim accents the hem.

She will make a beautiful statement in the wedding boutique that will be her home. I have been creating these custom pieces of art for two years now. My forms are featured in homes and business all over the country. This is what my recent customer had to say about the two forms she ordered:

Therefore, I may not be able to make your form look identical to the one in the photo. I will do my best to create a form that is similar to the form pictured in the listing, but I may have to substitute materials based on availability. I try my best to create a form that is a close to what my customer wants as possible. I do reserve the right to substitute materials and add new materials when necessary. I will advise you when a certain embellishment is no longer available to me and make every effort to replace it with something very similar.
