EYELASH HISTORY FORM (CHEAT SHEET)
-A 8 page write up of medications/ illnesses that will effect eyelash extensions and WHY! It tells why every question is important for both the tech and the client.
-Explains why accurate, asthma, blepharitis, cold sores, hormonal disorders, diabetes, cancer, ocular rosacea, stress, thyroid disease, trichotillomania, etc are important.
-After the eyelash history form for the client to sign (same document)

EYELASH HISTORY FORM (for client)
-A 5 page form filled with possible medications they may take, previous conditions or reactions and more! All for the client to fill out and sign.

EYELASH CONSENT FORM
-A single page write up stating the understandings and agreements for the appointment. (Client agrees to get extensions, understands the potential risks, understands the care instructions, understands they need to lay still and close eyes, maintenance instructions, etc)
-A separate place at the bottom for permission to take pictures
-Both a client AND tech signature required on form

CLIENT REFERRAL SHEET
-3 spots for clients name and 6 spots for who referred them
-A place underneath to write the dollar amount used or given (your choice)

EYELASH RECORD CARD
-2 pages (front and back but can be separated) that goes over the important things for a consultation. - ON THE FRONT: If they have had reactions before, extensions before, allergies, their desired style, their eye shape, type of makeup, notes, skin sensitivity date/ result, address (to know where your clients are coming from).
-ON THE BACK: The curl, thickness, lengths, colours and styles you will use to keep record for future appts, a visual to mark down what the set will look like, 6 rows to document the date they came in, the tech, the treatment, the charge and their signature approving the information.
-Both a client AND tech signature required on form
