* How to use:

1. You can print the file straight away & write down with pen/ pencil, or
2. Open PDF file in Adobe or your browser, fill in the blue boxes then print it out.

The blue boxes will not show up after printing.

* Download instructions:

1. After the payment has finished, the files will be sent to your email that connects with your Etsy account.
