This is a simple Puppy Birth Certificate designed to save you time. I found myself needing this at a local level from time to time and thought I'd share it to save you time, also. You can open the FILLABLE PDF with Acrobat or another PDF program. fill in and print on standard 8.5" x 11" paper and you're DONE! Also included is a jpg that you can print and fill manually if you prefer!

After your purchase, you will be able to immediately download a FILLABLE PDF file as well as a JPG image file 4950p x 3825p x 450dpi that can be printed on any standard printer with 8.5' x 11" paper loaded. Etsy will send a link to your email where you can download one or both files, but simply let me know if you have any problems and I'll get the files to you. There is a pink version in the shop, also.
