Just used this report sheet my last couple shifts and I really appreciate how it is organized. I have tried many different report sheets and they’re usually too cluttered and it’s hard to find the important information on them. This report sheet makes it easy to jot down info when receiving report and easy to reference when giving report. It’s also cute which is the cherry on top. Thank you!

I’ve been on the hunt for agreat report sheet since I worked in med/surg and after many months of planning and sketching out different versions, this is what I came up with. This version is more specialized to the CVICU. I hope you find it useful!
