This printable chart is designed to allow a child/individual who has been prescribed a sensory diet an easy way to write down activities for his/her sensory diet (each hour as directed) to be completed.

This chart may be used in several different ways but might I suggest:
1. Print and laminate the chart.
2. Write in activities to be completed as part of sensory diet.
3. Write a check mark, happy face, or picture of one's choosing in the circle provided to indicate that the sensory activity has been completed.

*Please be aware that while I try to ensure my image displays colors accurately, all monitors and systems differ. When an image is downloaded, it may be slightly different to what you see on your screen.

You can print and reprint the chart as many times as you like or print and laminate for easy re-use with dry-erase markers. Print on heavy cardstock for more durable use or regular printer paper (home computer or at a print shop). For best results, use your printer’s best settings and high-quality paper.
