None of us became therapists to do notes. That's why I created this therapy note template. I used to struggle writing my notes every week because it took too long and I didn't know if I was doing it correctly. When I became a Medicare and Medicaid provider, I researched ALL of their documentation requirements (and commercial insurance too!) and made sure to include all of those in this template...and it worked because using this template helped me pass a Medicaid audit!

A few things:
- This is a digital download available in .doc and .pdf (with fillable fields)
- You cannot download on mobile, only desktop
- You can edit info as needed
