1) monogram initials: FIRST name initial, LAST name initial, MIDDLE name initial
2) FULL LAST NAME. The LAST name initial will go in the middle of the monogram. Please provide full last name so there is no confusion about the order of the monogram.

Perfect! This is the second one I ordered. I love the style, and the cotton/ polyester material washes up nicely in cold water. Instructions say to not put the robe in the dryer, but I have been drying my first one on a low heat for years and it turned out well. It did not shrink. Thank you.
