This GG documentation spreadsheet includes the scoring guidelines, and it has been updated for the October 1, 2023 changes. I have successfully and repeatedly used the older incarnation of this form to prove GG performance to Humana, Cigna Health Springs, and State. Includes a second tab so you can also easily document prior level of function, what equipment the elder had used in their prior setting, and their section GG goals. Thoughtfully designed so you can clip it in to a binder without lopping off any print. Replace “River Chase Village” with your SNF name, make sure your margins are all set to “0” except the left margin- set that at “0.75”, and you’re all set.
