I'm so excited to share my Operating Room Report Sheet a.k.a. my OR brain that I use every day as an OR nurse myself! I have created a comprehensive report sheet that includes a quick patient overview section at the top, a section to document times in the OR, a section for common PreOp questions, a documentation section for IntraOp, and a section to help assist your handoff report to PACU with space to write down your specimens and any other notes at the bottom!

This PDF download includes one sheet that can fit up to 4 patients/procedures at a time (my personal favorite to use and fold into my scrub pocket), and one full-size report sheet if you only have one case to document or if you prefer a larger format! I've tailored this Operating Room Nurse Report Sheet specially for us perioperative/circulating nurses, and it has helped me streamline my workflow and stay organized throughout my shifts!
