This form helps you keep track of the necessary information regarding your client's contact information, including special dates.

This 8.5"x11" template is designed for anyone with experience working with photoshop layers, such as photographers. This template includes (1) fully customizable PSD file. Templates are created in Photoshop CS6 and are sized exactly to 8.5"x11" inches. There are no bleed areas or cropping marks. You will need basic knowledge of Adobe Photoshop to make changes to the templates.

PS Elements users will need to download and install an Ungroup Action to use this template, otherwise you will not have access to individual layers for changes. Layers are grouped to keep sections organized for easier editing. Please contact me if you prefer an ungrouped layered template or a .tiff file. I will be happy to accommodate your request at no charge!
