HOW IT WORKS
1. Fill in your name, team, month and year at the top.
2. Add one row per claim: customer name, claim number, carrier, dates and current status.
3. Update the status columns as claims move — use the same wording every time so managers can scan it.
4. Flag stalled claims (no movement in 10+ days) so your manager can help push them.
5. Sign and date the bottom and hand it in (or send a photo) when your manager asks — usually weekly and at month end.
