HOW IT WORKS
• Before a visit: five minutes with page 1.
• Before you leave, or right after: page 2, in the words used if you can.
• Every page has a "For" line and a "Last updated" line. Reprint a fresh set for every visit.
• Filled dots mark what to fill first. Hollow dots are optional; skip anything that doesn't apply.

WHAT YOU GET
• 4 PDFs, 2 pages each: US Letter and A4, in color and low ink
• Wide left margin for hole punching
• Print at home at 100% / actual size, as many times as you need. Fill in by hand (not a fillable PDF).
