HOW IT WORKS
1. Buy and download instantly from your Etsy account (Purchases & Reviews) — Etsy will also email you a link.
2. Open the PDF in a free PDF reader such as Adobe Acrobat Reader (desktop or mobile) to type into the fields and save. Some built-in browser or phone previewers may not save form fields, so Acrobat Reader is recommended.
3. Or print at home or at a print shop on standard paper and fill it in by hand. Print pages more than once if you need extra copies.
