Seller's description

A job hazard analysis that holds up, and eighteen of them already written.

Every GC wants a JHA, JSA, AHA, or pre-task plan before a crew starts a task, and most of what gets turned in is a blank grid filled out in the truck five minutes before the walk. This pack is a blank JHA form that asks the right questions, a 5 x 5 risk matrix with definitions in plain words, and eighteen pre-filled JHAs for the tasks a construction crew actually does, written in field language by a working steel erector, with the 29 CFR 1926 reference printed on each page.

WHAT'S IN IT, 29 PAGES PDF + EDITABLE WORD:
• One page on how to write a JHA that holds up: steps, hazards, controls in the order OSHA 3071 gives them (elimination, substitution, engineering, administrative, PPE), who signs, and when to redo it (new task, near miss, new crew, changed conditions)
• The blank JHA / JSA form, two pages: sixteen steps with Step / What can go wrong / How we keep it from going wrong, PPE checkboxes, permits, competent person, rescue plan, and a twelve-line crew sign-off. Three copies bound in
• A 5 x 5 risk matrix (likelihood x severity) with plain definitions for each level, what each score means for the job, and a before-and-after scoring worksheet. This is the "risk assessment template" page
• EIGHTEEN PRE-FILLED JHAs, one page each, same layout, steps and hazards and controls already written: concrete pour and finishing · trenching and excavation · scaffold erection and dismantling · ladder work · aerial and scissor lifts · roofing, low-slope and steep · structural steel erection · rebar placement · forklift and telehandler · crane pick and rigging · selective demolition · hot work · confined space entry · electrical rough-in and energized parts · masonry and concrete cutting (silica) · drywall and interior finish · housekeeping and material staging · working in heat
• Each pre-filled page has the 1926 reference line, PPE and permits pre-checked for that task, the competent person and rescue plan called out, two blank rows for your site-specific steps, and a crew sign-off block at the bottom
• A one-page blank in the same layout to copy for your own tasks
• A straight page on what this isn't: not a substitute for your competent person, not the GC's form if they require their own, not the written programs some of these tasks also need

THE WORD FILE. Same content, real tables, plain text. Put your company name in the header once and it's on every page. Delete a step, add a row, change a control to match how you actually do it. A pre-filled JHA you didn't change is a sign you didn't read it, and the Word file is how you change it.

WRITTEN STRAIGHT, NOT LAWYERED. Short sentences, field words. "Hose whips when the pump primes and hits the man holding it," not "struck-by hazard." Every citation checked against OSHA's posted text on the date it was built.

US Letter, print at any shop, grayscale-printer friendly. Instant digital download. One purchase covers your company: fill them out for every job, print all you need, hand the signed copy to the GC.

Pairs with the Site-Specific Safety Plan in this shop, which has three blank task hazard analysis sheets these pages fill, and with the 52 Toolbox Talks.

Template, not legal advice. It supports your company's safety program and the current OSHA standards, it doesn't replace them, and it doesn't replace a competent person looking at the actual job. You have to go through each page with your crew and change what's different on your site. Digital product, no refunds on digital files, but if something's wrong with your file, message me and I'll make it right.
