Every GC audit and every insurance visit asks the same two things about a crew: show me this man's orientation, and show me who was on site that day. Most outfits answer the first with a shrug and the second with a guess. This is the sheet that answers both. One orientation per person, every line initialed, both signatures at the bottom, into the training file. One visitor log at the gate with the rules printed on it, so the salesman and the inspector and the owner's brother-in-law all sign the same way.

Fourteen orientation lines to initial one by one: the safety plan and the competent persons, the emergency page, how to report an injury or near miss, stop-work authority, PPE, fall protection and rescue, the task hazard analyses, equipment they may and may not run, ladders and lifts, chemicals and the SDS binder, housekeeping and hot work, GC site rules, drug and alcohol policy, toolbox talks. Site specifics written in: muster point, nearest ER, competent persons, the language it was given in, interpreter, a buddy for the first week. The orientation list is the one from the Site-Specific Safety Plan in this shop (Section 11), cut out as a per-person sheet so the log and the orientation match line for line.
