I use a 100% cotton onesie (Gerber brand-size chart is below, please order based on that, as Gerber Onesies can run on the smaller side), and heat transfer vinyl, applied with a heat press.

For best results, I recommend turning the onesie inside out for washing and tumble dry on low.
