In the personalization box leave the name you would like on the worksheets. If you cannot see the personalization box (some mobile devices do not have this capability) please leave your information in the "Notes to INDIGO INK BOUTIQUE" Box before checkout. (This box is located under the listing image on the screen you choose your payment option). Or you can message your information to me via Etsy message.
