This listing is for one (1) bound carbonless custom business order form pad. This listing is NOT for a digital design. One pad holds 50 sets. A set consists of top copy white, bottom copy yellow. Whatever is written on the top copy or white sheet is automatically transferred to the bottom copy or yellow sheet without a carbon form.

We offer three sizes in both plys. Our Quarter size is 5.5" x 4.25", Half size is 8.5" x 5.5", and our full size is 11" x 8.5".

We offer a numbering option for both plys and all sizes. Numbering is where we place a number on each form set and can help you track your orders and invoices.

We understand purchasing a custom item can create a little anxiety. Please read through our process and allow us to ease some of that potential anxiety.
1. Look over our listings and choose the best ply and size for your business needs. We recognize this may be difficult to know. You are welcome to reach out if you have any questions on which size may be best for your needs.
2. Select the color and numbering option that works best for your needs.
3. If you would like, you are welcome to include your logo or graphic for your forms. Please provide a high quality jpeg or .png file that you have optained legal rights to use. We find using a high quality graphic provides the best results! Your logo can be attached to us in an Etsy convos (Etsy messaging). If you do not have a logo, no worries! We can work without logos as well!
4. Once you have determined the ply, size, color, and numbering options, please submit your order. We do not begin work on any orders until we receive your order with your selected preferences. When you place your order, please also submit any needs you may have for your forms or special fields you require. If you do not have enough room in the personalization box during checkout you can always send us an Etsy Convo with the information and/or your logo if you have one you would like to use. You are welcome to browse the graphics in our listings to view layouts we've created for other customers. Customer confidentiality is a must for us. Please do not ask us to copy the text from another customers forms to use as your own. Verbiage must be original to each customer.
5. Once we receive your order and preferences, we will add your order to our job queue. We process all orders in the order they are received. Please keep in mind, we typically have several orders at once and it may take us a couple of days to get to your order. We strive to get your order approved and processed within the timeframe provided; however, due to the custom nature of this product, delays can occur only because we want to make sure you receive the best quality product! It is an immense help if you check your Etsy Convos frequently to decrease any delays with your order.
6. We provide a proof for ALL customers purchasing a custom item. We DO NOT process ANY orders until we have received your final approval on your proof. We take this VERY seriously. We will ask you to look over your proof carefully to ensure everything is spelled correctly and is as you would like. We welcome your feedback on the proof because only YOU know how your form will be used and we want you to have exactly what you need for your business. We offer unlimited changes to the proof until we receive your final approval. Please know the turnaround time for this part of the process is typically very quick especially if you check Etsy Convos frequently. Please also note our shop is closed on weekends and holidays. Messages sent on weekends and holidays are not received until the next business day.
7. Once we receive your final approval, we will begin processing your order. This can take up to a week after a proof is approved and does not include shipping times. If you need your order within a certain timeframe or by a particular date, please communicate this to us. If it is sooner than the timeframe provided, we will ask you to pay our Rush Fee. Information regarding this can be found in our shop policies. Customers also have the option to upgrade their shipping which can also give us additional time to get your order to you by your deadline.
8. You will receive notification when your order ships. If you have any issues with your order once you receive it, please reach out to us immediately before leaving a review so we can have the opportunity to make it right with you. We strive for 110% customer satisfaction!
9. If you run out of forms, you can always place a re-order with us. We keep all files on hand for easy re-ordering. If you are looking to re-order with no changes to the original proof we provided to you, you can choose the re-order options in the dropdown. If you are looking to have changes done to the design, we would ask you to not use our re-order options. Our re-order options do not include our personalization fee. Re-order options are exclusively for those customers who have previously purchased from us and are looking to re-order their forms.
