A "parent interview" checklist.

I have been a family therapist in California for over a decade. I am sharing tools with you that I have developed , over years of trial and error , that helped me in my own practice.

The Parent Interview is what I call my 1st session with the parents/guardians of the minor client I will be working with. I created a one-page check-list that I have on hand for every Parent Interview. These are the questions/issues I want to address with the parents the 1st time we have a session to talk about counseling with their child. I use the check-list as a reminder and a way to stay on track during the parent session.

This is a simple one page check-list that I find helpful. I have the checklist in two forms so you can determine what works best for you.

You can laminate it or print on cardstock and keep it on hand during the parent session, or you can print copies and fill one out each session and add it to your client folder of notes--this is what I do.
