Simple printable forms for starting client work right

Lostmins shop makes printable business templates for freelancers and small service businesses. The goal is simple: help you start every new client relationship organized, with the right questions asked up front instead of scattered across emails and calls.

Our first template is a Client Intake Questionnaire, a clean 4-page printable PDF with 32 questions in 9 sections. It covers contact details, goals, scope, budget, timeline and how your client likes to communicate, so you can plan the project with everything in one place. Fill it in by hand or before printing.

Every form here is kept generic on purpose, so it works for any service business, not just one profession. More practical templates for running client work are on the way.
