Landmark Care Navigation exists because families facing healthcare crises deserve more than a Google search.

I am LaCinda Jones, a Licensed Clinical Social Worker with a Master of Jurisprudence in Health Care Law and over ten years of clinical experience in ICU and outpatient healthcare settings. Throughout my career, I watched families navigate some of the most consequential moments of their lives: a cancer diagnosis, a parent who refused help, a hospital discharging a loved one with no plan, a child with complex needs aging out of every system they knew, without the information they needed to do it well.

I created Landmark Care Navigation to change that.

Every guide in this shop was written by me, in plain language, for real families, not for healthcare professionals. They are not legal advice or medical advice. They are what I would hand you if you were sitting across from me in my office and asked, "What do I actually need to know?"
