Wallet Card with medication list, emergency contact, and medical information

I created this wallet card so you can easily carry your medication list and medical information with you at all times. This is a digital download- you will receive the PDF which is fully editable on your computer. Fill out your card then print. Cut the card out and fold it to credit card size. Note, this works best if printed on cardstock, though normal paper works fine as well.
